[J:COM Telemedicine (hereinafter referred to as this service)]

• To use it, you need to install a compatible device and designated app, create J:COM personal ID for each user, and register as a user. The communication environment (internet connection environment) and communication costs required for use will be provided at the customer's expense.
• Up to one contract per user.
• J:COM is the operating entity of this service and is not a party to contracts and delivery operations related to the provision of services by registered medical institutions/pharmacies, such as medical contracts and medication guidance.
• Registered medical institutions/pharmacies, or doctors, pharmacists, and other specialists affiliated with the medical institution/pharmacy are responsible for medical treatment, prescription, medication guidance, dispensing, and delivery of medicines, etc. to customers, and J:COM We do not provide services such as medical treatment or medication guidance to customers, nor do we prescribe, dispense, or deliver pharmaceuticals.
• J:COM is not responsible for any actions taken by customers using this service or their results.
• Limited to use in Japan.
• This is different from a health consultation service.

• If you request it, we will send you a separate document or electronic data containing the full text of the notice based on the Specified Commercial Transactions Act.

• To use it, you need to subscribe to a service specified by J:COM.
• Depending on the contract plan and usage environment, it may not be possible to provide.
• Available for properties that support interactive services. Even if you are already a subscriber, additional installation work (for a fee) may be required to use the interactive service.

• When using, canceling, or changing services for J:COM Telemedicine, installation costs/fees/initial costs/removal costs are required, except when using a smartphone.
• As of June 1, 2026 (with some exceptions).
• It may not be available depending on the address or building.
• Company names, product names, and service names are registered trademarks or trademarks of each company. Including some pending applications.
